| Miss Lisa’s Dance Studio Time of My Life Recital 2009 Order Form 1. Accounts must be at zero in order to perform in the recital. 2. We will be handing out costumes the week of April 20th-25th, 2009. 3. Students will wear their costumes to class the week of April 27th-May 2nd, 2009. Please bring other dance clothes to change in to. 4. Recital pictures will be taken here at the studio May 18th-May 21 and May 30th for Saturday classes. It is very important for everyone to attend pictures. The students will be photographed during their scheduled dance classes throughout that week for a class photo to hang in the studio then individual pictures for those who are interested. Order forms will be available soon. 5. Recital T-Shirts, flowers, and DVD’s will be available to purchase at the recital. Please send in your pre- orders now to ensure you receive one, there is a limited supply. 6. There is no video taping allowed at the recital. If you are caught video taping you will be asked to leave without refund. There is also no flash photography allowed during the shows. It is very distracting and dangerous to the performers up on stage. You will be able to take photos on stage after the show is finished. 7. Recital tickets will be handed out June 12th-15th. These tickets are included in your recital fee. If you need extra tickets you may purchase them at this time for an additional $7.00 each. Everyone 3 and under must have a ticket. Remember to bring your tickets to the show; no one will be let in without a ticket. 8. Dancin’ Dads and Dancin’ Moms sign up sheets are located on the recital information table in the lobby, please sign up now. Practices start in May. There is a $50.00 charge to participate. 9. Dress rehearsal is Friday, June 19th at Rickman Auditorium in Arnold, MO. Times will be posted on the information table soon. 10. Show times are as follows: Friday, June 19th 7:00pm (pre-show 6:30) Saturday, June 20th 2:00pm (pre-show 1:30) Saturday, June 20th 7:00pm (pre-show 6:30) 11. Fall 2009-2010 Schedule will be handed out in May. 12. Dance Magic Try-Outs are Saturday, June 6th. Anyone is welcome to attend. You must fill out an application, which is located on the recital information table, and return it with a $35.00 audition fee by May 16th, 2009. We look forward to seeing you at try-outs. 13. Please remember to check the recital information table often to stay up-to-date on information, dates, times, etc. Please fill out and return this portion by May 9th, 2009.
